You can purchase without setting up a PayPal account simply by selecting the “guest” option during checkout; however, if you are a frequent purchaser PayPal may require account setup.

Yes, tests are available to pick up onsite. Please coordinate with our staff for pickup at 1-800-549-1731 ext. 2. Our office is at 130 S High St, Sebastopol, CA 95472 across from the Wells Fargo parking lot.

Please be sure to double check your address, otherwise your kit will be returned to the sender.

If you ordered tests through our website then there is no need. If you obtained tests from a third party or have a promotional kit, be sure to include your name, email address, and phone number along with samples submitted so we can send you your results

Orders may be returned if the address provided is not correct. Please double check your contact information when placing an order and promptly notify us of any changes in address ASAP. Orders are processed within 24 hours excluding weekends.

If you need replacement cards or envelopes associated with a purchased kit, please email info@leafworks.com to request the number of replacements needed and whether you want standard priority or express mail shipping. If replacements are needed due to no fault of LeafWorks, pricing for replacements are listed below.

Envelope Replacement Fee:

  • $20 flat fee for up to 100 envelopes
  • $40 flat fee for 101-200 envelopes
  • $150 flat fee for 1000+ envelopes

Card Replacement Fee:

  • $5/card replacement

All replacements require the client to pay for shipping and indicate shipping preferences to place the replacement order. Otherwise, standard first class mail (3-5 business days) will be assumed. Invoice must be paid before replacements can be mailed.

LeafWorks is able to refund an order if an order cancellation is requested within 30 days of purchase and test kits received by the customer remain unopened and untampered; however, conditions apply. See below for terms and conditions.

Please contact info@leafworks.com to request order cancellation and include customer name, order number, copy of the Order Receipt and current mailing address. Then mail back any unopened, untampered test kits to:

All returns will be subject to a 5% return fee based on the amount of the original order to cover transaction and handling fees, and the customer is responsible for shipping costs. The customer can use the return label in the package.

If you want to return a partial order of unused and undamaged tests and your original order had bulk pricing applied, your original order will revert to the price applicable to the number of tests retained and any difference will also be deducted from your refund in addition to the fees above. Note in such circumstances where bulk pricing is removed, you may owe a balance on your final order and be required to pay to receive any results associated with the order.

You may use any unused return shipping label in your kit to submit returned unused tests; however, if you plan to send in a partial order in one shipment and unused tests in another, only one shipping label in a kit is provided and Customer must pay for additional shipments.

A refund will be mailed back to the customer upon receipt of the unopened untampered test kits.

If you have received an email that your samples have been received, please check the website page for the turnaround time for your purchased service. If we are outside that window, please ensure you have paid your bill. No results will be released without payment of an order in full without exception.
